

CNHC believes it is very important to care for vision, just as it is for other health aspects. One of the best ways to achieve optimum vision is by scheduling routine eye exams. Annual exams assist in the protection of your eyes, detection of any problems and treating those identified, at the earliest stage.
Appointments
Patients are seen by appointment. No referral is necessary from your primary care doctor to visit our vision center. Simply contact us to make an appointment.
CNHC accepts vision coverage from CareSource, Molina, Medicare, March Vision, Medical Mutual, and United Healthcare. Self-pay options are also available, including a sliding fee scale for patients without insurance coverage. A meeting with one of our billing specialist, at the time of appointment, will identify cost for our patients that need to utilize our sliding scale fee option.
How often do I need an eye exam?
CNHC recommends yearly exams to ensure ocular health, especially for those with diabetes and hypertension.
Corrective Lenses
We carry a variety of corrective lenses, available in both Adult and child sizes.

*Fashion tints are available as well as transition lenses and Anti-reflective coated lenses.
